I was given this after completing the quest Sinkeeper from Sinkeeper Mourana and answering the forgotten soul's question. It contained Discarded Medal of Valor as well as some vendor greys

Looting a Bell of Remembrance from a Sinstone Rampager (can drop from multiple mobs, also received one from a Nefarious Thug or Excavation Enforcer) for the first time starts Bell of Remembrance, which leads to Sinkeeper.

For Sinkeeper, you will receive a Bell of Remembrance. When used at the Seal of the Forgotten, it will summon a Forgotten Soul and you will have the option to either send it back into the crypt or send it to the Accuser for more judgment.

I sent mine back to the crypt because I didn't like the dialogue it offered me. That was the end for me.

So, curiosity got the best of me. I returned to Sinstone Rampager and killed them until I received Bell of Remembrance.

When I took this bell back to the crypt and used it, I sent the soul to the Accuser and received Satchel of Forgotten Heirlooms.

My first Satchel of Forgotten Heirlooms contained a single Discarded Commendation and a few trash items.

You can enter the crypt freely when the soul comes out, but you get the debuff Haunting Loneliness that will kill you if you are in there too long.

My covenant is Venthyr, I am unsure if that matters.

EDIT 03/28/2021: After doing this every day since I discovered how the Crypt worked, I've noticed a few additional things:

1. After you complete the initial quest from the first bell you receive, the repeatable portion only seems to be on a daily cooldown. I say this, because I receive my first bell of the day fairly quickly and I farmed once for a few hours killing mobs that I know drop it and I never got another.

2. If you read the ghost's message, you can choose the dialogue option appropriate to his message and Sinkeeper Mourana will say something to you based on what you chose:

If you choose the "wrong" one she will say "Unorthodox decision. I'll trust your judgment this time, <name>."
If you choose the "correct" one she will say "I deemed their fate the same. Excellent intuition, <name>."

3. It doesn't appear to matter which option you choose from the ghost's dialogue, both ways awarded me with a satchel.

4. Aside from the items I mentioned above, I have received Crumbling Sinstone, random greens and a few BoP epic items that seem to scale based on your current world quest item level.

I'll report back here if I get anything different.

I completed the quest, sending the soul to the Accuser, but just got 25 gold, no satchel.

I just got an ilvl 194 Vessel of Distilled Regret from a my bag along with the typical BOE green, grey vendor trash and Infused Ruby zone buff item.
